STEP 3: What Happens Next?

Once you have completed this form, we will send you an email activation link to the email address you provide (to confirm your address), followed by a comprehensive, medication safety report. Once activated, you can log in with your email address and chosen password to add more information to your profile and/or request that your doctor be copied on important communications.
