Question/Comment:

The fentanyl patch has been prescribed for my wife recently. The first patch took
some hours -- perhaps eight -- to have an effect. So far both the second and third
patches -- each replacing the prior patch at the 72 hour point -- have not had an
effect for hours. She's had to take Percocet in the interim between the time of
application of the patch and its effect. Is this the normal way that these patches
work? Thanks in advance for your response.

I Have also been on the patch for some time and I have found for me the first patch takes the most time to help you it can take up to 12 hours but the next patch you put on will not take that long because when you take of the first patch the drug is in your skin and it takes time for your body to absorb it so by the time your body is running out of the left over drug in your skin from the first patch the new patch is kicking in so aslong as you take them when your supposed to you should not have to wait for relief .

Hi maybe this will help, I have been on the patch for about 5 years, I too had a rough time finding a way to keep the pain in chcek when I would change the patch. Now I use 2 patches, say a 25 and a 50. I start with a 50, the next day I put on the 25, then every 3 days I instal a new patch according to which order it was installed, this way I always have medication going into my body. The trick is remembering which day I put on which patch.. I use 2 different coulered screws. (wood screws) brass for the 50 and nickel for the 25, Hope this helps, I know your grief. 60 yr old male.

Being in chronic pain is difficult to begin with yes it does take the first patch a long time to work (something most doctors fail to tell you) I have also found that the patches do not last a full 72 hours this is why my pain management doctor has me changing mine every 48 hours which seems to be helping me out much better then the 72 hour regimine. I do not know about the average pain patient, however I believe that most have to take an oral pain medication as well as the patch to stay out of pain, all I know is about is my case which is I have primary/progressive MS and I am wheelchair bound so I spend my life in pain however my new pain management regament has made life as it is tollerable for the first time in a very long time.

Hi,

Yes, this is the way these patches work. The different dosages peak at different times, meaning that her best day might be day 2 of 3 or day 3, or even day one for some. I don't know what dosage your wife was placed on, but they may have given her a low dose because the thing with this type of narcotic is that you have to be opiod tolerant. Now, in the last year or so, they've actually come up with a very low dose (low for opoids, still strong) of 12 mcg.
If after a week your wife is still having breakthrough pain, have her, or you if she's not able, to call her doc and let them know that her pain is breaking through and it's taking quite a long time for her to feel it's working. They may up her dose, or they may put her on a 48hr changeover instead of the 72 hr.
For me, when I was taking one dose of the patch I peaked on the 2nd day, what I called my good day. When I was upped to the next higher dose, my good day was still day 2 but day 3 was just terrible. That's when he switched me to the 48 hr dose. Much better without hiking the actual amount of medicine. If anyone is interested, you can find online the peak of each dose, and see how quickly the dose subsides to a level that may not cover your pain. I looked that up when the higher dose was not lasting that full 72 hrs. I compared that chart with the lower dose chart and it was all right there. No wonder I was having a terrible day before changing.

I HAVE HAD TROUBLE WAITING FOR THE PATCH TO TAKE EFFECT AFTER CHANGING, SOMETIMES IT TAKES 4 TO 6 HOURS TO FINALLY TAKE EFFECT AND DURING THE TIME PERIOD THE PAIN WOULD ESCALATE OUT OF CONTROL AND ADD THE WITHDRAWL SYMPTOMS TO IT AND THAT MAKES LIFE UNBEARABLE BUT I FOUND OUT BY MY PAIN MANAGEMENT DOCTOR THAT IF I TAKE AN EXTRA STRENGTH VICOTIN WHEN CHANGING THE PATCH IT TAKES A BITE OUT OF THE PAIN UNTIL THE PATCH KICKS IN. ASK YOU DOCTOR ABOUT THIS METHOD BECAUSE MY CHILDREN HATES TO SEE ME IN PAIN LIKE THAT EVERY 2 DAYS WHEN I CHANGE MY PATCH. I have been prescribed every type of fast acting, controled release and transdermal pain therepy medication known to man. I have MMD (myotonic muscular dystrophy) Duragesic has been wonderful for me. I can manage my pain levels enough to where I can play with my kids and maintain a semi-functional lifestyle. I also have to take OxyIR for break through pain relief. My doctor has found that you can change them out every 48 hours instead of 72 hours. This is for people who are tolerant of the drug and know the side effects and risks pertaining to it. Also, all Duragesic users go today and buy yourself a box of Tegaderm. It can be found at any pharmacy. It is the same adhesive used to hold and seal an IV line. (the clear thin plastic put over the IV site itself) It will hold the patch in place until the next patch is replaced. Also, go for the generics that DO NOT contain the gel. This is where people have had the most problems with accidental overdose. There are manufacturers that market the drug into the adhesive. This reduces the risk of damage to the patch itself and potential damage to yourselves. Hope this helps some of you. I was a Nurse/MedTech for 7 years before I fell ill. Please feel free to ask about your medications. I will try to be of some help to you or will help you find the answers you are looking for.

Please DO NOT adjust your dosages based on my doctors findings. You will need to discuss this with your pain management doctor to see if this option is available for you. Trying to "self adjust" dosing can not only lead to a potential ovedose, but can be a contract breaker between your doctors pain contract they have planned out for you. Use your medications as prescribed! This can help limit the need for premature dosage increases and over dependence. Again, I will try to answer any and all questions I can but, rememberthat your doctor must have the final say to maintain your health and safety.

I was on this med for 12 years. Being insured for that entire time I only became aware of the cost of this med towards the end of that 12yr period. My dosage was 225 micrograms every 48hrs. Most of the time it doesn't really last the full 72hrs. Average cost would have been $1500.00 per month!! If you lose your ins. can you pay for it??? Think, being 50 times stronger than morphine - you aren't going to get yourself off this drug without a real nightmare. While on this drug I was also on 100mg morphine every 12 hours and 20mg. compounded hydrocodone every 4-6hrs. 3yrs of this and I backed myself off the morphine first, 90 days later the same with the hydrocodone (10mg lortab without the acetaminaphine). For the next 9yrs I begged 2 primary care Drs to help AND my pain mgmt Dr. to help me get off the patch.

Oh, just in case you aren't warned off enough by now from this med., it will give you an insatiable desire for sweets, cereal, etc. My beautiful teeth that people had stopped me on the street and asked about, "who did you teeth" (they were my own, no bleeching, etc!) well, they were shot by the end, no, way before that 12 yr period, maybe 4yrs in they began a rapid deterioration. By the 8th yr. it was fairly complete. Now, tho I've owned several bus. and held many exec. positions over the years, people frequently mistake me for a former methamphetamine user or other drug user. I am now looking at having complete dentures. Apparently, I'm one of the early, regular patients who used this medicine ongoing. I am sure there will be many, many, many other horror stories regarding duragesic over the years. Diverticulitis........ well, I could just go on & on about the horrors of this medicine. I wish no one ever had to learn this horrible lesson on their own, but I know some will. God help you.

There have been several Law Suits out against this patch. My husband was on this and his body absorbed it so fast and he had sweat issues and other issues as well. This drug almost put him 6 foot under. He tried getting in on this Law suit but since we live in North Dakota the Law firm couldn't help us. He heard of the Suit threw television and in newspapers. So if doctors want to prescribe this med I am like "NO WAY!" My husband has tried many pain meds and other things like shots, accupunture, and chiropractors and became adictive to opiods and on other phsycotic meds that they thought he needed. Doctors kill more patients than pain meds! Like someone said on here, Doctors don't understand pain! My husband has had 8 surgeries for his back: 3 for discs, which the Doctor took one out but never replaced, two stimulators, operations to sew then to his spinal sack and sides. Now scar tissue has grown around bone and presses on nerves not to mention scar tissue hardens. If they were to operate the Doctor said he'd be worse off. He is on Morphine Sulfate and that has helped a lot! Cymbalta doesnt work for his pain but helps depression. I would be very careful using this patch, it is an awful one to get off of and can be dangerous!

I have been in considerable Spinal Stenosis and Lung Cancer pain for some time; consequently, a plethora of pain medications have been tried. In my case, I find that almost immediately I can tell whether or not the medication has efficacy. If it does not and/or it has some side effects, I terminate it's usage immediately.
Doctors know too little about pain , at least the majority of them, to insist that continue to try a medication that you as a patient know is simply not for you.
Too bad if it's a bit of trouble for the doctor to either prescribe another medication or adjust the dosage - that's precisely what you need them for. Don't allow yourself to be thought of as 'drug-seeking' or potentially addiction prone or problematic. You're the one with the pain and you deserve all the respect possible as a living, suffering human being.

I take fentanyl and it helps I alsotake percoert for the pain but when the time for the patch is do on the third day I know I need to change it. I get antisy or go in to a weird panic it's the same with percoert,But with either I could not stand the pain My doctor wanted to take me off all pain meds so he sent me to pain managrment they wantedn to put me on morphine ,I was in the Hospital am few mounts ago and the gave me morphine and i fliped out on them so i am glad i am not on that,but other than being hooked on what i take i have no other side effects.

I had some sort of reaction to this 4 years ago, that ended up making my overall pain much worse, and disabling me.

Has anyone ever heard of anything like this before?

I liked it the first 3 days, and was even thinking of asking the doctor of doubling the dosage, but then I started having some burning pain, (I initially dismissed this as one of my regular symptoms, it was Friday) then on Saturday I woke up feeling like I was about to come down with something, and I had my entire body feel like it was on fire burning from the bones out. I almost ended up calling an ambulance, then it eased up after 4 hours, so I waited, (My 2nd big mistake behind not ripping the patch off after the first sign of trouble) I had some easing of the severe burning for 2-3 hours, then the severe pain repeated, in sort of an on off cycle.
I also experienced, nausea, chills, sweating, and an instantaneous depression, the worst of my life, but I knew it was drug induced.
I told the doctor who just blew me off saying my body just might not be able to handle it, and that I was probably going through withdrawal from my old medication, and placed me on 3 times my old dosage, to which I felt nothing. I was in at minimum 3 times the pain I had been in the first place, only now nothing really helps. I also seem to have developed chronic fatigue like symptoms , where I have to crash after doing a task for anywhere from 30minutes to an hour, I could work 2 hours tops before nodding off.

The depression passed after 3 weeks of me having to rationally convince myself it would disappear, and the burning and fever like symptoms slowly cycled out over the duration of a month, but the decimated stamina, as well as my now constant worsened pain level have been consistent ever since November of 2004 when this happened, I was fired in early 05, and just recently gotten on disability, but I had to go on Medicaid, (now Medicare) and start over with all of my doctors, and none of them believe that I am in the kind of pain I am in.

I don't know what else to do. has anyone ever heard of anything like this where a medication seems to have induced a permanent change in the one using it?

Yes, I was permantly damaged by the drug synthroid. Within a few weeks all of my teeth had loosened, twisted, and fallen out, also darkened later on the surface which I could not remove while still in my mouth!!! I happened to see a television program from doctors telling that there were 5 drugs one should always be aware of when prescribed. I do not remember what the others were for when I found out the synthroid I had started taking was among, I immediately turned all attention to that drug. The doctors told that synthroid (for thyroid) affects all of the body's systems by removing calcium that each system needs to function properly. Then the doctors said that the above tooth lost which I describe could happen. There should have been a huge blackbox warning about this from the manufacturer and the doctor who prescribed this drug to me should have warned me about this and let me decide if I wanted to take the risks. And to top it all off, I've just found out that this synthroid was prescribed to me after the manufacturer had lost TWO CLASS ACTION SUITS!!! MANY THANKS FOR YOUR WARNING AND DESCRIPTION OF SIDE EFFECTS YOU HAD FROM FENTANYL!!!

I could get myself down to 150-175micrograms of the Duragesic patch, but NEVER could get lower than 150micrograms. When I finally found the ONLY way anyone I ever talked to got off of it (and this includes many, many hundreds of hours of internet research) it involved going to California and spending $15,000.00 cash. By the grace of God, I had the money - at that time, barely. Ultra Rapid Detox. They put me under anesthesia for a few hrs and cleansed my liver, blood, etc. By the time I awoke I was pretty much thru the pain part of withdrawl, but not the 24-36hour worst bout of diareah (spelling) you can imagine, nor was I thru with the nausea. I don't care what anyone tells you - it took me almost 5 days before I could eat ANYTHING. A week later I was back @ home and finally free of fentanyl, after 12 YEARS! And I don't care what dosage you're on - it will eventually not give you pain relief, you will have to increase the dose. The floor charge nurse where I was in the hospital told me, in his opin. this stuff truelly was the 'devils own', much worse than anything he'd ever seen with heroin. So I say to everyone, think very carefully before you let your Dr. put you on this med.

Fentanyl has made my pain management more tolerable. I have no stomach upset and function better on a daily basis. For me the main is still there but endurable, I no longer grimmace, grind my teeth nor cry out of frustration.

The patch works for me if I take Vicoden for break through pain. I wish I had know about that. I get irritated skin where I put the patch, almost as though I am allergic to the adhesive. Some of the time I have to use extra medical tape to keep them on. The heat (outside) really does a number on the adhesive too. I have been on the same dose for over 6 years now and my doc doesn't want to up the doseage but I am having to take more break through meds. Has this happened to anyone else? Other than those problems, it does work for me.

I have just started on Fentanyl and when my husband and I read your comment I was shocked because I also experience the same issues you do. In handling the adhesive problem, always use a cotton ball of alcohol and swab the site clean, let the area dry, then apply the patch without touching it as much as possible. IE: I use the plastic to slip the patch onto the skin. If your skin is moist you could try dusting the area with some baby powder. Also, the rashes that will soon become sores are hard to control. I have wore other medical patches with the same results that you are getting. Try to never place the patch over the same area for a couple weeks. I wear this patch on my fore arms and I haven't lost one yet.

I also get the same physical relief that you describe. After 20 years of severe pain I'm relieved to not have something tearing my stomach up. I was on Percocet for over 15 years, As for pain control it was the ONLY thing that could control my pain and leave me with some sense of normal function. Like clean house and fix dinner and still make it to my bed without falling over in misery. Like you, I'm also suffering severly at this time and found that after getting Darvocets post dental surgery recentIy I felt alot better. The Fentanyl is only good as a constant level of pain relief, however in no way does this drug solve the pain issue. My dr. prescribed Torodol IM (injection form) and I only used it when the pain was out of control. It worked great. Today, I had to stop that (Torodol) drug due to an upcoming biopsy, so now I'm told that at least for two weeks the only thing I'll get is Fentanyl.

I'm also 51 and female. I'm gonna try user female2. Should we discuss our script strengths? I also had to sign papers promising to return all used patches. Is this common?

If you are in need of a long term pain medication the fentanyl patch is a convenient option available. The patches last for 72 hours so you are provided round the clock medication and do not have to worry about taking pills at precise times to maintain relief levels. Minimal side effects experienced by me. Overall, I would reccomment the use of this product.

If they are using the name brand product Duragesic, and not any of the generic patches and they have trouble keeping the patch on to contact Janssen and let them know. You may have to be firm with the customer service representative, depending on who answers. With a recent proof of purchase they will send you a product called "Bioclusive" It is a clear, waterproof latex bandage and it covers the entire patch to keep it secure. The medical tape and other things I tried did not work. Do not hesitate to call Janssen. It is worth it, and it will save you the cost of buying these products out of your own pocket. Go to a medical store and price these covers. None of the discount mass merchandise stores carry them in my area. As of August 2007, the covers cost $1.99 plus tax for just one cover. Estimate you will use 125 for a year and it adds up to over $250 a year, on top of the price of the prescription! Pratice applying the cover on your body without the patch at first if you are unfamiliar applying this type of bandage. Take it from someone that learned the hard way and got a patch so twisted up I could not use it and it was a waste of the patch. Also, if you use the name brand of Duragesic, check their website. You can get coupons with very few restrictions to offset the cost. Right now, it is $50 off, and with my insurance coverage, it is cheaper for me to get the name brand instead of generic. Hope this information helps someone!

Use something like cover roll stretch tape to keep the patches on. It is very flexible and not very irritating so that the fentanyl patches will not bother you as much if you have sensative skin. Without the tape the patches can bother me around the edges for some reason the tape over the edge seems to hold it down better and less irritation. I can also see the date I wrote on the patch better because the tape is white.

I hope you do not feel like I am insulting you because I used to do the exact same thing to my patches until I found out it was unsafe.
The patient information & instructions that come inside Ortho-McNeil /Janssen's Duragesic patches have bold print, stating
"Never cover a Duragesic patch with any other bandage or tape."
If applying first aid tape only around the patch edges does not keep the patch on, Janssen suggests using Tegaderm or Bioclusive adhesive dressings to cover and secure patches. I use Bioclusive # 2463, it securely covers a 100mcg patch and holds it securely for 72 hours through baths, showers and life in general, and it includes a sticker you can write your next patch change date on. But if you have latex sensitivity you should avoid Bioclusive. Good luck.

Please use a calendar to keep track of the days when you have to change the patch! It is so much easier to do if you do that. You may lose track of what day to change it if you don't. It is easier to put the patch on your upper arms or stomach. If you put it on your back you may need someone to help you. Also keep medical tape on hand, they say it is waterproof BUT sometimes it does come loose around the edges. If you go out in the sun for a lengthy time make sure your patch is covered, the sum may cause it to release the medication sooner then it should. I went to Jamaica and made sure it was under my swim suit and I had no problem with it releasing the meds or with it coming off. Make sure no one touches or handles it.

I have no problems with the medication that are worth talking about just the adhesive on the patch, very often mine crinkle up and the sides and ends turn under making it hard to keep the patch in place.

You need to be aware that Duragesic can be addictive and if you ever need to come off the medication for whatever reason, you will need to go down very very slowly and that it may be difficult to and a length of time to do so.

Just that it may help to relieve some of your pain in order to make it more manageable, but that you run the risk of depression, lack of energy and loss of energy when using it. That is something you must consider and weigh.

Be sure to brush your teeth afterwards, the sugar in the lozenges can cause dental caries. Also, keep the lozenge against your cheek! Do not suck like a lollipop and DO NOT CHEW IT!

The best thing I have found to help adhere the patch to your skin is to place a layer of Tegaderm over it. It will last until the patch needs to be removed. Tegaderm can be found at your local pharmacy. Tegaderm is the same clear sealing adhesive used to hold nd cover IV tubing. Hope this helps others. this is the only way I can keep my patches on. lol
